Job summary

Evolve Therapy Services, LLC in Clarion, PA is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility. You will plan, organize, and direct occupational therapy services to ensure high-quality resident care.

Responsibilities include screening residents, developing treatment plans, coordinating with an interdisciplinary team, supervising COTAs, and ensuring accurate documentation and billing. Full-time or PRN positions with competitive benefits.

Qualifications

• Must hold a degree in Occupational Therapy and an active OT license in practicing state.
• Continuing education to maintain licensure and ongoing professional development required.

Responsibilities

• Screen residents to determine appropriate interventions and treatments.
• Evaluate residents and develop short- and long-term goals and methods.
• Interpret evaluation findings and communicate plans to residents, families, and team.
• Establish therapy schedules in collaboration with the team.
• Conduct therapy per plan and document and bill services.
• Perform additional duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement compliance.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
