Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Youngstown, OH. The OT will plan, organize, and direct therapy services, ensuring compliance with state and federal standards and high-quality resident care.

Responsibilities include screening residents, developing therapy plans, coordinating with the care team, documenting and billing, supervising COTA, and maintaining licensure through continuing education.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention plan.
• Evaluate residents and develop short- and long-term treatment goals.
• Communicate evaluation findings and treatment plans to residents and families, and the interdisciplinary team.
• Schedule resident therapy in coordination with team members.
• Document and bill OT services per treatment plan.
• Perform duties to effectively treat patients and support clinical outcomes.
• Oversee COTA and ensure documentation and reimbursement practices are accurate.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
