Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Warren, PA. This PRN position focuses on planning, organizing, and directing Occupational Therapy services to ensure high-quality resident care as directed by the Administrator.

Responsibilities include screening, evaluating, scheduling, therapy delivery, documentation, and supervision of COTA.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best OT interventions and goals.
• Evaluate residents to develop and update treatment plans and goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Create individual therapy schedules with team coordination.
• Deliver OT services with proper documentation and billing per plan.
• Perform other duties as needed to support patient care.
• Oversee COTA as needed and ensure compliant documentation and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility
Job Type: PRN
Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
