Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Clarion, PA. The role centers on planning, directing Occupational Therapy Services in accordance with federal, state, and local standards to ensure the highest quality resident care.

This full-time position offers growth opportunities and requires an active OT license in the practicing state, with ongoing continuing education encouraged and collaboration with an interdisciplinary team.

Qualifications

• Must have an OT degree and an active license in the practicing state.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents to inform and develop treatment plans and goals.
• Communicate findings to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Deliver therapy per plan and manage documentation and billing.
• Perform other duties to effectively treat patients as needed.
• Oversee COTA and ensure documentation and reimbursement practices are followed.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
