Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in South Range, WI. This PRN role focuses on planning, directing occupational therapy services in alignment with current standards to ensure high-quality resident care.

You will screen residents, develop treatment plans, and coordinate with families and the interdisciplinary team. Documentation, scheduling, and supervision of COTA are required as part of delivering effective therapy

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine the best intervention.
• Evaluate residents to inform and develop treatment plans; set goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish individual therapy schedules with other team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.

Education

Active Occupational Therapy license in WI

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
