Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Johnstown, PA. This PRN position focuses on planning, directing, and delivering OT services to residents according to regulatory standards.

You will conduct screenings, develop treatment plans, and coordinate with the interdisciplinary team to ensure quality resident care and appropriate documentation and billing.

Qualifications

• Assessment of residents to identify OT needs and goals.
• Developing individualized treatment plans and goals.
• Communicating evaluation findings to residents, families, and the care team.

Responsibilities

• Screen residents to determine best OT intervention.
• Evaluate residents to inform treatment plans and goals.
• Interpret and communicate results to residents, families, and team.
• Establish therapy schedules with the care team.
• Provide therapy and ensure proper documentation and billing.
• Perform other duties as required to treat patients.
• Oversee COTA and ensure compliant documentation and reimbursement.

Occupational Therapist for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
