Job summary

Concierge Home Care is seeking a Home Health Occupational Therapist (OTR) to join our Orange Park, FL team with a flexible schedule and pay-per-visit options.

As an OT in home health, you will conduct evaluations, develop treatment plans, and educate patients and caregivers while coordinating with physicians and the care team. A Florida OT license, CPR certification, and reliable transportation are required; options include full-time or PRN positions.

Qualifications

• Current Florida OT license
• Valid driver’s license and reliable transportation
• CPR certification
• Experience supervising COTAs preferred
• Home health experience helpful

Responsibilities

• Conduct patient evaluations and home environment assessments
• Develop individualized treatment plans based on physician orders
• Improve functional mobility and activities of daily living
• Educate patients and caregivers on adaptive strategies and safety
• Collaborate with physicians and interdisciplinary care teams
• Supervise COTAs when appropriate

Your Role as a Home Health Occupational Therapist

• Conduct patient evaluations and home environment assessments

• Collaborate with physicians and interdisciplinary care teams
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate
