Job summary

Santa Cruz Post Acute is seeking a licensed Occupational Therapist to join our dedicated rehabilitation team. We offer full-time, part-time, and PRN opportunities with a starting rate of $50–$58/hour depending on experience and DOE.

You will evaluate, plan, and implement therapy programs to help residents regain independence, collaborating with an interdisciplinary team to deliver high‑quality, evidence‑based care in a supportive environment.

Qualifications

• Licensed OT in California (OTR/L) with active registration.
• Excellent interpersonal and documentation skills.
• Experience in SNF preferred but not required; new grads welcome.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide direct therapeutic interventions to improve ADLs and mobility.
• Document evaluations and progress notes per regulatory standards.
• Collaborate with nursing, therapy staff, and physicians.
