  • Supervise COTAs and support staff.
  • Document in medical records and communicate with physicians and families.

This full-time opportunity is ideal for an OT who is passionate about helping residents achieve maximum independence, function, and quality of life in a skilled nursing and rehabilitation setting.General PurposeThe Occupational Therapist (OT) evaluates and treats residents, develops and implements individualized treatment plans, communicates with physicians, families, and interdisciplinary team members, and maintains accurate and timely documentation in the medical record. The OT also provides clinical guidance and education to staff to support optimal resident outcomes.Essential Duties and ResponsibilitiesEvaluate residents within 24 hours (or one business day) of physician referral; document delays appropriatelyDevelop individualized treatment plans and obtain physician approval for servicesProvide occupational therapy treatment per the physician-approved plan of careSupervise Certified Occupational Therapy Assistants (COTAs), aides, and students in accordance with the state practice actCommunicate resident progress, challenges, and plans with the rehab and interdisciplinary teamsParticipate in patient care conferences, utilization review meetings, and rehab meetings as neededComplete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirementsEducate residents, families, and caregivers on safety, adaptive techniques, and home programsParticipate in discharge planning and coordination of careRecommend and facilitate ordering of durable medical equipment (DME) to promote functional independenceConduct in-services and training for facility staff as assignedReport equipment issues promptly to ensure safe and effective operationMaintain professional, respectful relationships with residents, families, and staffSupervisory ResponsibilitiesSupervise Certified Occupational Therapy Assistants (COTAs), aides, and studentsQualificationsEducation & ExperienceBachelor’s degree in Occupational Therapy requiredMaster’s or Doctorate degree in Occupational Therapy preferredSkilled nursing or geriatric experience preferred but not requiredLicensure & CertificationsCurrent, active California Occupational Therapist license in good standingMust meet all continuing education requirements per state regulationsSkillsProficient in electronic documentation systems (Casamba and PointClickCare preferred)Strong clinical judgment, communication, and organizational skillsAbility to work independently and as part of a collaborative teamPhysical RequirementsFrequent standing, walking, pushing, pulling, and reachingAbility to lift and move up to 50 pounds frequentlyOccasional stooping, kneeling, crouching, and balancingVisual and auditory acuity sufficient for patient care and documentationReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsWork EnvironmentSkilled nursing and short-term rehabilitation settingNoise level typically low to moderateOne primary work locationWe are an equal opportunity employer.
