Responsibilities

  • Evaluate residents and develop individualized treatment plans.
  • Provide skilled OT services focused on improving ADLs.
  • Supervise and collaborate with COTAs.
  • Coordinate care with physicians, nursing staff, residents and families.
  • Participate in care conferences, rehab meetings and discharge planning.
  • Educate residents and caregivers on goals, home exercise programs, safety, and fall prevention.
  • Recommend adaptive equipment and durable medical equipment (DME).
  • Maintain accurate, timely, and compliant clinical documentation.
  • Contribute to quality improvement initiatives and staff education programs.
