Job summary

Santa Cruz Post Acute is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our therapy team in a skilled nursing environment. Roles include working under an OT to deliver ADL, mobility, strength, and cognitive programs with daily notes and progress updates.

The facility supports flexible schedules (FT/PT/PRN) and offers a comprehensive benefits package. California licensure and ACOTA program graduation are required; SNF experience is preferred but new grads are welcome

Qualifications

• Current California COTA license.
• Graduate of an accredited COTA program.
• Strong communication, teamwork, and documentation skills.
• SNF experience preferred but new grads welcome.

Responsibilities

• Provide treatment interventions as outlined in the resident’s individualized OT plan of care.
• Assist in implementing therapeutic programs to improve ADLs, mobility, strength, coordination, and cognitive function.
• Document daily treatment notes, progress updates, and resident responses according to facility and regulatory standards.
• Collaborate with the occupational therapist, therapy staff, nursing team, and other interdisciplinary members to support resident goals.
• Educate residents, families, and caregivers on therapeutic exercises, safety techniques, and functional strategies.
• Maintain compliance with state and federal regulations and professional standards.
