Santa Cruz Post Acute in Santa Cruz, CA is seeking a licensed Occupational Therapist (OTR/L) to join our rehabilitation team. We provide patient-centered care in a collaborative environment where your clinical skills and compassion can thrive.

Qualifications

• Licensed Occupational Therapist (OTR/L) in California.
• Graduated from an accredited Occupational Therapy program.
• Strong interpersonal, communication, and documentation skills.

Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans for residents.
• Provide direct therapeutic interventions to improve ADLs, mobility, strength, and independence.
• Document evaluations, treatment notes, progress reports, and discharges per facility standards.
• Collaborate with nursing, therapy staff, and physicians for coordinated resident-centered care.
• Educate residents and families on safe techniques and home-care strategies.
• Participate in care conferences and interdisciplinary planning.
