Job summary

Human Touch Home Health is seeking a compassionate Physical Therapist (PT) to join our growing Washington, DC team on a PRN basis. You’ll provide home health PT services, create individualized treatment plans, and empower patients to regain independence in the comfort of their own homes.

You’ll enjoy flexible scheduling, professional independence, and meaningful patient impact as you help patients move forward one visit at a time.

Qualifications

• Active Washington, DC Physical Therapist License Required.
• Valid Driver's License and Reliable Transportation.
• Home health experience and Oasis familiarity preferred.

Responsibilities

• Complete physical therapy evaluations, reassessments, and discharge visits.
• Develop individualized plans of care based on patient goals and clinical needs.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on exercises, mobility training, fall prevention, balance, and home safety.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians and the interdisciplinary care team.
• Complete timely clinical documentation using WellSky.

What You'll Do

• Active Washington, DC Physical Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Interpersonal Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
