Job summary

Montgomery Nursing & Rehabilitation Center in Montgomery, NY, is seeking a licensed Occupational Therapist to provide PRN rehabilitation services within a 100-bed skilled facility.

You will evaluate geriatric patients with complex medical and functional needs, develop individualized treatment plans, and work with an interdisciplinary team to deliver coordinated, patient-centered care. This role emphasizes mobility, ADLs, and fall prevention.

Qualifications

• Must hold a current NY OT license and registration.
• Must carry malpractice insurance.
• Demonstrated compassion in caring for elderly residents.
• Experience in rehabilitation and gero settings is preferred.
• Strong communication and teamwork abilities.

Responsibilities

• Evaluate and treat geriatric patients with mobility, cognitive, and functional needs.
• Develop individualized treatment plans to improve independence.
• Collaborate with nurses, physicians, SLPs, and PTs for coordinated care.
• Provide adaptive equipment training and implement fall prevention strategies.
• Educate residents, families, and staff on care plans and progress.

General Job Summary:

• Licensed Occupational Therapist (OT) providing PRN (as-needed) rehabilitation services in the Facility.
• Occupational Therapist (OT) is evaluating and treating geriatric patients with complex medical, cognitive, and functional needs.
• Occupational Therapist (OT) develops individualized treatment plans to improve mobility, daily living skills, and overall functional independence.
• Occupational Therapist (OT) collaborates with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care.
• Skilled in adaptive equipment training, fall prevention strategies, and patient/caregiver education.
• Flexible and responsive, providing high-quality care across variable schedules and patient acuity levels

Requirements

• Must possess a current license and registration to practice as a Occupational Therapist, OT in the State of NY.
• Must possess current malpractice insurance, license in good standing
