Job summary

Putnam Nursing & Rehabilitation Center, a 160-bed skilled facility in New York, seeks an Occupational Therapist (OT) to join our rehabilitation team. The OT provides PRN or full-time services, evaluates and treats geriatric patients, develops individualized plans, and collaborates with nurses, physicians, speech and physical therapists to improve ADLs and independence.

Qualifications

Responsibilities

• Provide PRN (as-needed) rehabilitation services in the facility
• Evaluate and treat geriatric patients with complex medical, cognitive, and functional needs
• Develop individualized treatment plans to improve mobility and ADLs
• Collaborate with nurses, physicians, speech and physical therapists to deliver coordinated care
• Provide adaptive equipment training, fall prevention strategies, and patient education

Putnam Nursing & Rehabilitation Center is a 160-bed Skilled Nursing Facility hiring for both *Full-Time* & *Per Diem* Occupational Therapists, OT who will work closely with other rehabilitation staff and the interdisciplinary care team to help residents improve activities of daily living (ADLs), strength, and independence. For those benefit eligible we offer Aetna Health Insurance that begins the 1st of the month after hire!!!! Low employee contributions!

Per Diem Applicants MUST have SNF experience. For the Full-Time position both new graduates and experienced candidates are encouraged to apply!

General Summary of Key Responsibilities

• Licensed Occupational Therapist (OT) providing PRN (as-needed) rehabilitation services in the Facility.
• Occupational Therapist (OT) is evaluating and treating geriatric patients with complex medical, cognitive, and functional needs.
• Occupational Therapist (OT) develops individualized treatment plans to improve mobility, daily living skills, and overall functional independence.
• Occupational Therapist (OT) collaborates with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care.
• Skilled in adaptive equipment training, fall prevention strategies, and patient/caregiver education.
• Flexible and responsive, providing high-quality care across variable schedules and patient acuity levels

Requirements

• Must possess a current license and registration to practice as a Occupational Therapist, OT in the State of NY.
• Must possess current malpractice insurance, license in good standing
• Dedicated to providing extraordinary compassionate care as a Occupational Therapist, OT
• Energetic, enthusiastic, committed, & passionate about quality care
• Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents
