Job summary

Putnam Nursing & Rehabilitation Center in New York is seeking an Occupational Therapist to join our rehabilitation team. The OT will provide PRN and full-time coverage, evaluating and treating geriatric patients with complex medical and functional needs, and developing individualized treatment plans to improve mobility and activities of daily living.

Qualifications

• Must possess a current license and registration to practice as an Occupational Therapist in NY.
• Must possess current malpractice insurance, license in good standing
• Passionate about providing high-quality compassionate care for residents

• Covid-19 vaccinations are encouraged but not mandated

Responsibilities

• Licensed OT providing PRN (as-needed) rehabilitation services in the Facility.
• Evaluating and treating geriatric patients with complex medical, cognitive, and functional needs.
• Develop individualized treatment plans to improve mobility, daily living skills, and independence.
• Collaborates with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care.
• Skilled in adaptive equipment training, fall prevention strategies, and patient/caregiver education.
• Flexible and responsive, providing high-quality care across variable schedules and patient acuity levels

Putnam Nursing & Rehabilitation Center is a 160-bed Skilled Nursing Facility hiring for both *Full-Time* & *Per Diem* Occupational Therapists, OT who will work closely with other rehabilitation staff and the interdisciplinary care team to help residents improve activities of daily living (ADLs), strength, and independence.

Per Diem Applicants MUST have SNF experience. For the Full-Time position both new graduates and experienced candidates are encouraged to apply!

Requirements

• Must possess current malpractice insurance, license in good standing
