Job summary

City View Post Acute in San Francisco, CA seeks a licensed Occupational Therapist (OT) to join our in-house rehabilitation team. We offer Full-Time, Part-Time, and PRN schedules with competitive hourly pay ($56-$62+/hour, DOE).

Experience welcomed, new grads encouraged to apply. You will evaluate residents, create individualized treatment plans, and provide skilled interventions to improve daily living, mobility, and independence.

Qualifications

• Current California license or eligibility to practice as an Occupational Therapist (OT).
• Strong clinical and communication skills are required.
• Excellent interpersonal and organizational abilities are essential.

Responsibilities

• Perform comprehensive evaluations and assessments of residents.
• Develop individualized treatment plans to improve ADLs, mobility, strength, and balance.
• Provide skilled OT interventions to enhance independence and quality of life.
• Educate residents and families on therapeutic exercises and safety strategies.
• Document evaluations, treatments, and progress in medical records.
• Collaborate with physicians, nursing staff, PTs, SLPs, and the care team.
• Maintain resident-centered, compassionate care and professional conduct.

What You'll Do

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled interventions designed to improve function, independence, and quality of life.

Responsibilities include:

• Perform comprehensive occupational therapy evaluations and assessments.
• Develop individualized treatment plans based on resident needs, goals, and functional abilities.
• Provide skilled therapeutic interventions to improve activities of daily living, strength, coordination, balance, mobility, and functional independence.
• Address cognitive, physical, and functional limitations that impact residents' ability to participate in daily activities.
• Provide training in adaptive techniques, energy conservation, safety strategies, and use of adaptive equipment.
• Educate residents and families regarding therapeutic exercises, safety, functional mobility, and strategies for increasing independence.
• Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
• Collaborate with physicians, nursing staff, Physical Therapists, Speech Therapists, and the interdisciplinary care team.
• Accurately document evaluations, treatments, progress notes, and discharge planning.
• Participate in care planning and interdisciplinary meetings as appropriate.
• Maintain a compassionate, professional, and resident-centered approach to care.

Qualifications

• Current California Occupational Therapist (OT) license or eligibility for licensure.
• Strong clinical, communication, and problem-solving skills.
