Job summary

City View Post Acute in San Francisco, CA is seeking an Occupational Therapist (OT) for Full-Time, Part-Time, and PRN opportunities. This in-house rehab team focuses on improving residents' independence and quality of life.

You will evaluate residents, develop individualized treatment plans, and provide skilled interventions to enhance daily activities, strength, balance, mobility, and cognition. Collaboration with physicians, nurses, PT, and speech therapy is essential.

Qualifications

  • Current California OT license or eligibility for licensure.
  • Strong clinical, communication, and problem-solving skills.
  • Excellent interpersonal and organizational skills.

Responsibilities

  • Perform comprehensive occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident needs, goals, and functional abilities.
  • Provide skilled therapeutic interventions to improve ADLs, strength, coordination, balance, mobility, and independence.
  • Address cognitive, physical, and functional limitations impacting daily activities.
  • Educate residents and families on exercises, safety, and independence strategies.
  • Collaborate with physicians, nursing staff, PTs, SLPs, and care team.
  • Accurately document evaluations, treatments, progress notes, and discharge planning.
  • Participate in care planning and interdisciplinary meetings.

Pay: $56–$62+/hour, DOE

What You'll Do As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled interventions designed to improve function, independence, and quality of life.
  • Perform comprehensive occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident needs, goals, and functional abilities.
  • Provide skilled therapeutic interventions to improve activities of daily living, strength, coordination, balance, mobility, and functional independence.
  • Address cognitive, physical, and functional limitations that impact residents' ability to participate in daily activities.
  • Provide training in adaptive techniques, energy conservation, safety strategies, and use of adaptive equipment.
  • Educate residents and families regarding therapeutic exercises, safety, functional mobility, and strategies for increasing independence.
  • Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
  • Collaborate with physicians, nursing staff, Physical Therapists, Speech Therapists, and the interdisciplinary care team.
  • Accurately document evaluations, treatments, progress notes, and discharge planning.
  • Participate in care planning and interdisciplinary meetings as appropriate.
  • Maintain a compassionate, professional, and resident-centered approach to care.
Qualifications
  • Current California Occupational Therapist (OT) license or eligibility for licensure.
  • Strong clinical, communication, and problem-solving skills.
  • Excellent interpersonal and organizational skills.
  • Passion for rehabilitation and resident-centered care.
