Job summary

City View Post Acute in San Francisco, CA is seeking an Occupational Therapist (OT) to join our in-house rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities with competitive pay of $56-$62+ per hour, DOE.

New graduates are welcome and mentored. As an OT, you will evaluate residents, develop treatment plans, and provide interventions to improve daily living activities, strength, balance, and independence.

Qualifications

  • Current California Occupational Therapist (OT) license or eligibility for licensure.
  • Strong clinical, communication, and problem-solving skills.
  • Excellent interpersonal and organizational skills.
  • New graduates are welcome and encouraged to apply!

Responsibilities

  • Evaluate residents and develop individualized treatment plans.
  • Provide skilled interventions to improve activities of daily living, strength, balance, and independence.
  • Document evaluations, progress notes, and discharge planning accurately.
  • Collaborate with physicians, nursing and therapy colleagues and the interdisciplinary care team.

What You'll Do

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled interventions designed to improve function, independence, and quality of life.

Responsibilities include:

  • Perform comprehensive occupational therapy evaluations and assessments.
  • Develop individualized treatment plans based on resident needs, goals, and functional abilities.
  • Provide skilled therapeutic interventions to improve activities of daily living, strength, coordination, balance, mobility, and functional independence.
  • Address cognitive, physical, and functional limitations that impact residents' ability to participate in daily activities.
  • Provide training in adaptive techniques, energy conservation, safety strategies, and use of adaptive equipment.
  • Educate residents and families regarding therapeutic exercises, safety, functional mobility, and strategies for increasing independence.
  • Monitor resident progress and modify treatment plans as appropriate to maximize outcomes.
  • Collaborate with physicians, nursing staff, Physical Therapists, Speech Therapists, and the interdisciplinary care team.
  • Accurately document evaluations, treatments, progress notes, and discharge planning.
  • Participate in care planning and interdisciplinary meetings as appropriate.
  • Maintain a compassionate, professional, and resident-centered approach to care.
Qualifications
