Job summary

Pride Health is hiring an Occupational Therapist - Home Health to support a healthcare facility in Fernley, NV. Schedule is 09:00 AM to 05:00 PM, Monday to Friday, with a 13-week contract length and potential extension. Pay ranges from $60.00 to $63.00 per hour, depending on experience and credentials.

Qualifications

• Current state OT license required.
• CPR certification required.
• Valid driver’s license required.
• Reliable transportation and auto insurance required.
• Minimum 2 years OT experience in acute or rehab setting required.
• Graduate of an accredited OT program with a BS in OT.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong clinical assessment, documentation, communication, and organizational skills.

Responsibilities

• Conduct comprehensive clinical assessments and identify patient needs.
• Develop and update Plans of Care, progress notes, and orders.
• Provide interventions to improve ROM, strength, coordination, mobility, and ADLs.
• Evaluate performance across self-care, work, leisure, and rest.
• Recommend and maintain adaptive equipment and hand splints; educate use.
• Collaborate with physicians and care team to coordinate care.
• Follow infection control and universal precautions.
• Maintain patient records and timely documentation.
• Educate patients and caregivers to support goals and independence.
• Maintain and troubleshoot OT equipment.
• Ensure compliance with payer, regulatory, licensing, and organizational standards.

Pride Health is hiring a Occupational Therapist - Home Health tosupport our client's healthcarefacility in Fernley, NV.

Job Details

Schedule: 09:00 AM to 05:00 PM, Monday to Friday

Location: Fernley, NV 89408

Float / Coverage Area: Within 60 miles

Job Type: Local contract

Contract Length: 13 Weeks (Potential for an extension)

Pay Range: $60.00 to $63.00/hour*

Mileage Reimbursement Rate (if applicable): standard IRS rate.

*Pay offered is based on experience,expertise, credentialing, and education.

Duties

• Conduct comprehensive clinical assessments to identify patient needs and determine appropriate occupational therapy interventions.
• Develop, document, and regularly update individualized Plans of Care (POC), progress notes, physician orders, and other required clinical documentation.
• Provide therapeutic interventions to improve ROM, strength, coordination, functional mobility, safety, and activities of daily living (ADLs).
• Evaluate occupational performance across self-care, work, leisure, and rest activities, including physical, sensory-motor, cognitive, and psychosocial factors.
• Recommend, fabricate, and maintain adaptive equipment and hand splints; educate patients on appropriate use and safety.
• Collaborate with physicians, caregivers, and interdisciplinary healthcare professionals to coordinate patient care and communicate patient progress.
• Follow infection control practices and universal precautions to maintain a safe patient-care environment.
• Maintain accurate patient records, analyze clinical data, and ensure timely completion of documentation.
• Provide patient and caregiver education to support treatment goals, functional independence, and continuity of care.
• Perform basic cleaning, setup, troubleshooting, and maintenance of occupational therapy equipment.
• Maintain compliance with applicable payer, regulatory, licensing, and organizational requirements.

Requirements

• Current state Occupational Therapist (OT) license - Required.
• Current CPR certification - Required.
• Valid driver's license - Required.
• Reliable transportation and current auto insurance - Required.
• Minimum 2 years of Occupational Therapist experience in an acute care or rehabilitation setting - Required.
• Graduate of an accredited Occupational Therapy program with a Bachelor of Science degree in Occupational Therapy.
• Experience with Homecare Homebase (HCHB) is required/preferred.
• Strong clinical assessment, documentation, communication, and organizational skills.
