Job summary

BoldAge PACE in Crestwood, IL is seeking an Occupational Therapist to join the interdisciplinary team. You will plan, organize, and implement OT services for participants, conduct assessments, and develop treatment plans to support independence at home.

Responsibilities include home visits, coordinating adaptive equipment and home modifications, teaching participants and caregivers, ensuring proper documentation, and mentoring COTAs.

Qualifications

• Master's level degree in Occupational Therapy.
• Current license to practice OT in Illinois.
• Two years experience in an interdisciplinary team preferred.

Responsibilities

• Conduct initial and periodic in-person assessments to evaluate functional abilities and home environment.
• Develop, implement, and revise individualized treatment plans.
• Deliver OT interventions across settings including home, clinic, and PACE center.
• Educate participants and caregivers on equipment use and home safety.
