Job summary

BoldAge PACE in Florida is seeking an Occupational Therapist to join the interdisciplinary team delivering participant-centered care in home and clinic settings. You will assess, plan, and implement OT services, coordinate with caregivers, and help participants maintain independence.

Responsibilities include initial and ongoing assessments, developing treatment plans, educating participants and families, supervising COTAs, and collaborating in IDT meetings to ensure seamless care coordination.

Qualifications

• Advanced degree in Occupational Therapy
• Current state license to practice OT
• 2 years of experience on an interdisciplinary team preferred
• 1 year of frail/elderly population experience preferred

Responsibilities

• Conduct initial and periodic in-person assessments to evaluate participants’ functional abilities and home environment.
• Develop, implement, and revise individualized treatment plans.
• Deliver OT interventions across settings including center, clinic, and home.
• Educate participants and caregivers on equipment use and home safety.
• Supervise COTAs and provide mentorship to staff.
• Participate in daily IDT meetings and coordinate care across teams.
