The Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team. You will assess patients, develop treatment plans, establish goals, document progress, and educate families in therapy methods. Experience with long-term or geriatric care is preferred.

Responsibilities include supervising assistants, maintaining HIPAA-compliant records, and collaborating with cross-disciplinary teams to deliver high-quality patient care. Equal opportunity employer.

Qualifikationen

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.

• Assess referred patients to determine rehabilitation needs and establish goals.
• Develop and administer treatment programs based on patient capacity and tolerance.
• Document patient condition, progress and discharge plans; educate patients and families.
• Collaborate with other disciplines to provide patient-centered care.
