Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitative team. The OT will assess referred patients, implement treatment plans, and set goals aligned with each patient’s capacity under physician direction.

The role requires strong documentation, collaboration with disciplines, and adherence to confidentiality. Prior long-term or geriatric care experience is preferred; supervision of OT Assistants is expected, with duties including assessments, progress

Qualifications

• Graduated from an accredited OT program and licensed to practice.
• Oversee and supervise OT Assistants; collaborate with other disciplines.
• Experience in long-term/geriatric care preferred.
• Strong documentation and patient-centered care; adhere to confidentiality.

Responsibilities

• Assess referred patients needing rehab and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient status; educate patients and families on therapy methods.
• Ensure timely reports, progress notes, and discharge documentation.
