Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join a dedicated rehabilitation team. The role focuses on assessing patients, creating and implementing treatment plans, and documenting progress under physician direction.

You will educate patients and families, coordinate with the care team, and supervise Occupational Therapy Assistants while maintaining professional standards and confidentiality. The position offers growth within a long-term care setting.

Qualifications

• Graduated from an accredited OT program and holds a valid state license.
• Oversees Occupational Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Excellent documentation and patient record writing.
• Strong collaboration with all disciplines.
• Professional and punctual with confidentiality observance (HIPAA).
• Ability to develop and document treatment plans with goals.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and implement individualized treatment plans.
