Job summary

Continuum Therapy Partners is seeking an Occupational Therapist for PRN daytime work at Heritage Health Services in Port Washington, WI. You’ll join a team that values flexible scheduling while delivering high-quality patient care.

The role requires a Master’s or Doctorate in Occupational Therapy, an active state license, and NBCOT certification. Prior OT experience is preferred, and you’ll educate patients and families on safety and adaptive equipment.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required


• Current, unencumbered state Occupational Therapy License; NBCOT Certification required


• 1 year of previous experience as an Occupational Therapist preferred


• Evaluates and develops individualized treatment plans to improve patients' functional independence


• Documents patient progress and modifies treatment plans based on clinical outcomes


• Educates patients and families on adaptive equipment and home safety recommendations
