Job summary

WelbeHealth is seeking an Occupational Therapist to coordinate restorative therapy within an interdisciplinary team, delivering assessments and interventions to maximize participant independence and safety both in facility and home settings.

You will participate in IDT meetings, develop care plans, and document outcomes while guiding therapy assistants in Functional Maintenance Programs. A NJ OT license and CPR/First Aid are required or imminent.

Qualifications

• Bachelor’s degree in occupational therapy; master’s preferred.
• Valid New Jersey Occupational Therapy License, or proof of license eligibility.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct Occupational Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs
• Work with Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
