Job summary

WelbeHealth seeks an Occupational Therapist to join a Participant care team within our PACE program. You will conduct assessments, participate in IDT meetings, deliver and document interventions, and focus on independence in ADLs for our participants.

You’ll work with Therapy Assistants to implement Functional Maintenance Programs, with a balanced schedule and no after-hours documentation. Compensation includes base pay plus bonus; WelbeHealth provides comprehensive healthcare, a 401(k) match,

• Assessment and intervention skills in occupational therapy.
• Ability to work within an interdisciplinary team.
• Commitment to patient-centered care and ADLs improvement.

Responsibilities

• Conduct Occupational Therapy assessments in facility and home as needed.
• Actively participate in IDT meetings to develop patient care plans.
• Deliver and document interventions per care plans.
• Provide active/passive exercise, gait, and ADL training.
