Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist for a full-time role in a skilled nursing facility. The position emphasizes one-on-one patient care, independent treatment plan development, and collaboration within a clinical team to optimize recovery outcomes.

You will evaluate patients, design evidence-based plans, and provide hands-on therapy while educating patients and caregivers on home programs and prevention.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
