Job summary

Total Care Therapy LLC, a therapist-owned rehabilitation company, is hiring an Occupational Therapist in Westerville, OH. Join a clinician-first team providing OT services in assisted living and independent living communities.

The role evaluates residents, develops individualized treatment plans, and collaborates with an interdisciplinary team to maximize independence for older adults. Compensation is competitive; full-time schedule with weekdays.

Qualifications

• Current State OT license.
• Current CPR/BLS certification.
• Valid driver's license with reliable transportation and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance and independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers and facility staff on safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated patient-centered care.

About the Role As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
