Job summary

Total Care Therapy, a therapist-owned rehabilitation company, seeks an Occupational Therapist to evaluate residents and create individualized plans to improve independence in daily living, mobility, cognition, and safety.

You will deliver skilled OT interventions, supervise COTAs, and ensure timely EMR documentation while collaborating with an interdisciplinary team to deliver patient-centered care in central Ohio.

Qualifications

• Current State OT license required.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide OT interventions to improve ADLs, IADLs, cognition, balance and safety.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as needed.
• Supervise COTAs per state regulations and document in EMR.
• Educate residents, caregivers, and staff on safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
