Job summary

Total Care Therapy in Ohio is seeking an Occupational Therapist to evaluate residents and develop individualized treatment plans that improve independence in activities of daily living, upper extremity function, cognition, and safety.

This part-time, per-visit role offers flexible scheduling at Mt Vernon and Zanesville sites, with supervision of COTAs, home health experience preferred, and a therapist-owned culture focused on clinician support and career growth.

Qualifications

• Current OT license required in the state of practice.
• CPR/BLS certification current and valid.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide OT interventions to improve ADLs, IADLs, and cognitive function.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely EMR documentation and communicate with the team.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
