Job summary

Total Care Therapy is seeking an Occupational Therapist to join our therapist-owned team serving assisted living and independent living communities in Northeast Ohio. The OT will evaluate residents, develop plans to improve ADLs, cognition, and safety, and provide skilled interventions to boost independence.

We value clinician-led setting, mentorship, and opportunities for growth. Enjoy flexible scheduling with a part-time Monday–Friday pattern; weekends available but not required.

Qualifications

• Current State OT License
• Current CPR/BLS Certification
• Valid driver's license with reliable transportation
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as needed.
• Supervise COTAs in accordance with state regulations.
• Complete timely documentation using EMR.
• Educate residents, caregivers, and staff on adaptive techniques and fall prevention.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
