Job summary

The VNA Health Group is seeking a full-time Occupational Therapist to provide evaluation and treatment to patients in their homes or community settings in Essex County, NJ. The role emphasizes collaboration with physicians and care teams to develop and implement individualized plans of care.

Candidate should have NJ OT licensure and 2–3 years of clinical/community experience, strong computer and communication skills, and a valid NJ driver’s license.

Responsibilities

• Provide occupational therapy evaluation and treatment to patients in their homes or community settings.
• Develop and implement treatment plans in collaboration with physicians.
• Document changes in patient condition and update care plans/outcomes.
• Identify education needs for patients, caregivers and families and deliver instruction.
• Monitor quality and appropriateness of services provided.
• Participate in QAPI and in-service programs.
• Adhere to infection control and safety policies.
• Comply with attendance and appearance policies.

Position Summary:

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings

Principal Responsibilities:

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.

• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician

• Document all changes in client’s condition and updates care plans and outcomes accordingly

• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies

• Monitor quality and appropriateness of services provided

• Participate in VNAHG’s QAPI and in-service programs

• Adhere to agency infection control and safety policies

• Adhere to the organization’s policy in regard to absenteeism and appearance

Job Competencies & Minimum Qualifications:

• Occupational Therapy Licensure in NJ

• 2-3 years of prior clinical experience and community-based experience preferred

• Strong computer skills

• Strong communication and interpersonal skills

• Valid NJ driver's license

Working Conditions/Physical Demand

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
