VNA Health Group in Middlesex County, NJ is seeking an Occupational Therapist for a full-time role to provide home-based OT evaluations and treatments to patients in their homes or community settings.

Responsibilities

• Provide occupational therapy evaluation and treatment to patients in their homes or community settings.
• Assess and evaluate clients, develop and implement action plans with physician collaboration.
• Document changes in client condition and update care plans and outcomes.
• Identify education and counseling needs for patients, caregivers and families, and provide teaching strategies.
• Monitor quality and appropriateness of services and participate in QAPI and in-service programs.
• Adhere to infection control and safety policies and absenteeism/appearance policies.

Position Summary:

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings.

Principal Responsibilities:

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
• Document all changes in client’s condition and updates care plans and outcomes accordingly.
• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies.
• Monitor quality and appropriateness of services provided.
• Participate in VNAHG’s QAPI and in-service programs.
• Adhere to agency infection control and safety policies.
• Adhere to the organization’s policy in regard to absenteeism and appearance.

Job Competencies & Minimum Qualifications:

• Occupational Therapy Licensure in NJ
• 2-3 years of prior clinical experience and community-based experience preferred
• Strong computer skills
• Strong communication and interpersonal skills
• Valid NJ driver's license

Working Conditions/Physical Demand:

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
