The VNA Health Group is seeking an Occupational Therapist to provide evaluation and treatment to patients in their homes or community settings in Somerset County, NJ. This is a full-time role aimed at delivering high-quality, compassionate home health care.

Responsibilities include collaboration with physicians, documentation of care plans, and ongoing participation in education and quality programs. Eligible candidates hold NJ OT licensure, 2–3 years of experience, strong communication skills,

Qualifications

• NJ Occupational Therapy License required.
• 2–3 years of clinical and community-based experience preferred.
• Strong computer skills.
• Strong communication and interpersonal skills.
• Valid NJ driver’s license.

Responsibilities

• Support organization’s mission by striving for excellence with positive interpersonal relations.
• Assess and evaluate clients for occupational therapy needs and develop and implement appropriate plan of action in collaboration with the physician.
• Document all changes in client’s condition and updates care plans and outcomes accordingly.
• Identify patient, caregiver, and family counseling and education needs and plan teaching strategies.
• Monitor quality and appropriateness of services provided.
• Participate in VNAHG’s QAPI and in-service programs.
• Adhere to infection control and safety policies.
• Adhere to the organization’s absenteeism and appearance policies.

Position Summary

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings

Principal Responsibilities

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician
• Document all changes in client’s condition and updates care plans and outcomes accordingly
• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies
• Monitor quality and appropriateness of services provided
• Participate in VNAHG’s QAPI and in-service programs
• Adhere to agency infection control and safety policies
• Adhere to the organization’s policy in regard to absenteeism and appearance

Job Competencies & Minimum Qualifications

• Occupational Therapy Licensure in NJ
• 2-3 years of prior clinical experience and community-based experience preferred
• Strong computer skills
• Strong communication and interpersonal skills
• Valid NJ driver's license

Working Conditions/Physical Demand

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000 When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
