Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to evaluate and treat patients in a Skilled Nursing Facility setting, following an individualized care plan. You will contribute to patient progress, educate patients and families, and work closely with facility staff to ensure high-quality care.

The role includes developing plans of care, setting functional goals, re-evaluating progress, and maintaining accurate documentation.

Qualifications

• OT license or eligibility in Illinois.
• Master's degree in Occupational Therapy required.
• Experienced in SNF settings is a plus.

Responsibilities

• Evaluate patients and develop plans of care with measurable goals.
• Provide treatment and educate patients on progress and outcomes.
• Re-evaluate patient progress and revise plans of care as needed.
• Complete timely documentation including evaluations and progress notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Collaborate with facility staff to ensure excellent care for you patients


• Evaluating new patients, developing plans of care and setting measurable, functional goals


• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes


• Re-evaluating patient progress, revising plans of care and resetting goals


• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
