Job summary

Enhance Rehab is seeking a qualified Occupational Therapist to join our team and provide evaluation and treatment to patients in a skilled nursing facility. You will follow an individualized plan of care and collaborate with facility staff to ensure excellent patient outcomes.

You will supervise Occupational Therapy Assistants, conduct re-evaluations, and maintain timely documentation while educating patients and families about progress and prognosis.

Qualifications

• Licensed OT with SNF experience or willingness to work in SNF settings.
• Ability to develop and implement measurable goals for functional progress.
• Strong documentation and progress note writing skills.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Collaborate with facility staff to ensure high-quality patient care.
• Evaluate new patients, develop plans of care and set measurable goals.
• Treat patients, address concerns, and educate patients on their injury, progress and outcomes.
• Re-evaluate patient progress, revise plans of care and adjust goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
