Job summary

Enhance Rehabilitation is currently seeking qualified Occupational Therapist candidates to join our dynamic team. You will evaluate and treat patients in a SNF setting following an individualized plan of care.

As part of our growing team, you will provide excellent patient care, collaborate with facility staff, document evaluations and progress, and supervise Occupational Therapy Assistants to ensure high-quality outcomes.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Provide excellent care and customer service to patients and family members.
• Collaborate with facility staff to ensure excellent patient care.
• Evaluate new patients, develop plans of care and set measurable functional goals.
• Treat patients, address concerns and educate them on injury, progress and outcomes.
• Re-evaluate patient progress, revise plans of care and reset goals.
• Complete timely and accurate documentation including evaluations, reevaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
