Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team. In this role you will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will collaborate with facility staff, develop plans of care with measurable goals, and provide education to patients and families about injuries, progress and outcomes. You will also document progress and supervise Occupational Therapy Assistants to ensure high-quality care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
