Job summary

Innovate Rehab and Wellness in Salisbury, MO is seeking a Licensed Occupational Therapist (OT) for a full‑time position in a skilled nursing facility. The schedule is Monday–Friday with flexibility, and there may be a split between Salisbury and Moberly; PRN at one facility.

You will evaluate patients, develop and adjust individualized treatment plans, and collaborate with PT, SLP, physicians, and the care team to restore function and independence.

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred (new grads welcome).
• Strong communication and problem-solving skills.
• CPR certification preferred.

Responsibilities

• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.
