Azza HealthCare Agency is seeking an Occupational Therapist to provide one-on-one therapy to older adults in private residences, senior living communities, and other settings, focusing on mobility, independence and quality of life.

Provide

Responsibilities include conducting evaluations, developing individualized treatment plans, delivering evidence-based therapies, and documenting care within electronic medical records.
Collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists, and educate patients, caregivers, and staff to promote safety and aging in place.
Flexibility to incorporate telehealth and virtual care enhances access and continuity of care.
Qualifications include a valid OT license, degree from an accredited program, computer proficiency, and CPR certification.
Physical requirements involve exerting medium effort, assisting with patient transfers, performing therapeutic activities, and driving to various locations.
