Job summary

Oasis Therapy Services is seeking an Occupational Therapist to provide one-on-one rehabilitation at Eden Vista Litchfield in Litchfield, Minnesota. This PRN opportunity offers flexible scheduling and the chance to deliver personalized care in an independent living, assisted living, and memory care community.

You will evaluate residents, develop individualized OT plans, and educate residents and caregivers. The role requires Minnesota OT licensure, NBCOT, and CPR, with Net Health EMR

Qualifications

• Active Minnesota OT license.
• NBCOT OTR certification in good standing.
• Graduate of ACOTE-accredited OT program.
• Current CPR certification.
• Geriatric or senior care experience preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans.
• Provide one-on-one treatment focusing on activities of daily living, functional mobility, safety, and independence.
• Monitor residents' progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Direct and supervise Occupational Therapy Assistants as appropriate.
• Complete accurate Net Health documentation and collaborate with the care team.
