Lakeport Post Acute in Lakeport, CA is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team. We welcome both full-time and part-time opportunities with a competitive hourly rate starting at $65.00 and DOE.

Work in a supportive environment with a new Director of Rehabilitation and a focus on resident-centered care. Responsibilities include evaluating residents, developing individualized treatment plans, and delivering OT services to improve ADLs and independence.

Qualifications

• Current California OT License is required.
• Graduate from an accredited OT program.
• Strong clinical assessment, treatment planning, and documentation skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on functional goals.
• Provide OT services to improve independence with ADLs.
• Implement evidence-based therapeutic interventions and adaptive strategies.
• Assess cognitive, physical, and functional abilities to maximize patient outcomes.
• Collaborate with physicians, nurses, therapists, and interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment plans and home programs.
• Maintain accurate documentation in compliance with regulations.
• Monitor progress and modify treatment plans to achieve optimal outcomes.
