Job summary

Total Care Therapy is seeking an Occupational Therapist to join our clinician-owned team serving seniors in Granger, Indiana. You will evaluate residents and develop individualized plans to improve independence in ADLs, cognition, and safety.

This part-time, per-visit role offers flexible scheduling in assisted living and independent living communities, with weekends optional. We value hands-on mentorship, supportive leadership and opportunities for professional growth.

Qualifications

• Current State OT license.
• CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Experience in geriatrics or assisted living preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled OT interventions to improve ADLs/IADLs, upper extremity function, balance, cognition and safety.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely and accurate documentation using EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques and safety.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
