Job summary

Total Care Therapy in Richfield, OH is seeking a Part-Time Occupational Therapist to evaluate residents and develop individualized treatment plans that enhance independence in daily living, upper extremity function, cognition, and safety.

You will provide skilled OT interventions, assess adaptive equipment and home modifications, supervise COTAs as required, document in the EMR, and collaborate with an interdisciplinary team to deliver patient-centered care in assisted and independent living

Qualifications

• Current State OT License
• CPR/BLS Certification
• Valid driver's license and reliable transportation
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
