Job summary

Total Care Therapy is seeking an Occupational Therapist to evaluate residents in Johnstown, OH, and create individualized treatment plans to improve independence in ADLs, cognition, and safety. The role involves working within assisted living and independent living communities, with opportunities for collaboration and professional growth.

You will supervise COTAs, document care in the EMR, and educate staff and residents on safety and energy conservation.

Qualifications

• Current State Occupational Therapist (OT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
