Job summary

Total Care Therapy is seeking a licensed Occupational Therapist to join our Cincinnati team, serving assisted living and independent living residents.

You will evaluate residents, develop individualized OT treatment plans, address ADLs, balance, cognition, and safety, and collaborate with a interdisciplinary team to improve quality of life for older adults.

Qualifications

• Active State OT license required.
• Current CPR/BLS certification.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, balance, cognition, and safety.
• Assess and recommend adaptive equipment and home modifications.
• Reassess progress and adjust treatment plans as needed.
• Supervise COTAs in compliance with state regulations.
• Complete timely documentation using the EMR.
• Educate residents, caregivers, and staff on safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated patient care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
