Job summary

Total Care Therapy is seeking a compassionate Physical Therapist for our Granger, IN teams serving assisted living and independent living residents. You will evaluate residents, design individualized treatment plans, and deliver therapy to improve mobility and independence.

Qualifications

• Current Indiana State PT license.
• Current CPR/BLS certification.
• Valid driver's license with reliable transportation.
• Experience in geriatrics/home health/assisted living or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention and safe mobility techniques.
• Collaborate with interdisciplinary team to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.

• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.

• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
