Job summary

Total Care Therapy seeks a dedicated Occupational Therapist to assess residents in Fishers, IN, and craft individualized plans to improve daily living activities and safety. You’ll collaborate with an interdisciplinary team to maximize independence and quality of life for older adults.

Qualifications

Responsibilities

• Evaluate residents and develop individualized OT plans.
• Provide OT interventions to improve ADLs, IADLs, cognition, balance and safety.
• Assess and recommend adaptive equipment and home modifications.
• Reassess progress and adjust plans as needed.
• Supervise COTAs per state regulations.
• Document care using EMR.
• Educate residents and staff on safety and energy conservation.
• Collaborate with interdisciplinary team for coordinated care.

Setting: Assisted Living and Independent Living Communities Available Locations:

Fishers South and Carmel Split

Compensation: Per-Visit or Salary (based on years of experience) Scheduling: Monday-Friday Full-Time (Weekends available but not required)

About the Role As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

Evaluate residents and establish individualized occupational therapy plans of care.

Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.

Assess and recommend adaptive equipment and environmental modifications.

Reassess patient progress and modify treatment plans as appropriate.

Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.

Complete timely and accurate documentation using the electronic medical record (EMR).

Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.

Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

Current State Occupational Therapist (OT) License

Current CPR/BLS Certification

Valid driver's license, reliable transportation, and auto insurance

Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
